How to refer a patient to our office

In order for an insurance company to pay a massage therapist, the patient needs to have a prescription from their doctor.

You can use your prescription pad or you can use the form to the right. If you use your prescription ,we need to have the following information on the script:

    • The modality (Massage Therapy, Manual Therapy,         etc.)
     • The diagnosis code
     • The number of treatments
       or frequency and duration
     

You can fax the script to us at 850-432-6815 or the patient can bring it with them to the office.
